Because of the nature of the web, sometimes stuff like this is unavoidable. When you go to the page and don't submit for a while, my system has no way of knowing if you're still looking at the page and thinking of words or if you've left. This is the purpose of locking, but sometimes what feels like reasonable play can be interleaved with someone else's attempt to play and give strange results. The best advice I can give is to submit bogus words every minute or so to be sure to retain the lock.

The second issue sounds like it is probably caused by your browser's or ISP's cache (that retains recently viewed pages so that it doesn't need to fetch them from the internet when you visit them again). You shouldn't be caching the pages, but some ISPs and browsers don't do it right. If it happens, try hitting reload; that might help. If you don't think it's the cache and have a way to reproduce the problem, let me know!
